Your Opportunity:

The Transition Services Coordinator (TSC) - Acute Care screens, assesses, coordinates, and evaluates the needs of patients and families who cannot function independently in the community. The TSC expedites discharges for individuals who require community services. This involves extensive assessment, facilitation and negotiation to develop the plan of discharge, and complete the electronic community referral for Continuing Care Living Options. The TSC must have excellent knowledge of community agencies/resources in order to facilitate the linkages to appropriate services.

Description:

As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values of AHS. PLEASE NOTE: Provisional Permit holders will be paid $40.76 to $55.79 per hour. Upon obtaining a full practice permit, the rate of pay will be adjusted to the applicable Registered Nurse rate in accordance with UNA Article 25.02(b)(i) or (ii), as applicable.

Required Qualifications:

Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP) certification. If you have not practiced as a Registered Nurse within the past five (5) years, completion of the following free courses is required prior to employment: CRNA Medication Management (Chapters 1-4): https://connect.nurses.ab.ca/home/learning-and-development/learning-modules NextGenU Health Assessment Resources Certification: https://courses.nextgenu.org/course/view.php?id=271.

Additional Required Qualifications:

Minimum of recent 5 years Pediatric experience required.1-2 years of Pediatric Experience with Transitional Services. Must have excellent assessment, interpersonal, communication, and organizational skills in order to work with a multi-disciplinary team which includes allied health, physicians, and nursing staff. Physically able to perform assigned tasks which may include walking a minimum of 3kms per shift (including stairs) while carry resource materials weighing up to 25lbs. Required to perform up to 6 hours of keyboarding (up to 40wpm) and written documentation per shift while navigating a variety of software programs.

Preferred Qualifications:

Baccalaureate Degree in Nursing preferred; however, skills and experience relevant to nursing practice in community sector will be considered. Community related experience preferred i.e. Integrated Home Care of Integrated Supportive and Facility Living experience. Caseloads and daily work can be unpredictable-frequent interruptions, multi-tasking required and the ability to reset priorities on an hourly basis due to challenges i.e. capacity triggers.
