Baccalaureate degree in Nursing. Current registration with CARNA in good standing. Knowledge and experience in community nursing, complex discharge planning, and at least two years of supervisory experience required.
Additional Required Qualifications:
Must have an understanding of appropriate placement of patients and the appropriate supports that may be required. Experience analyzing data, identifying trends and make related recommendations which contribute to decreasing length of stay or readmission to an acute care facility is required.
Preferred Qualifications:
One year of community care coordination experience preferred. Master’s Degree in Nursing preferred.
