Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).
Additional Required Qualifications:Two years of recent experience preferably in a Community Health and Acute Care settings. Experience working with geriatric, mental health or complex care adult populations. Previous experience working with an interdisciplinary team. Current successful completion of Healthcare Provider CPR program is required. Ability to work independently with limited supervision. Proficient use of computer applications.
Preferred Qualifications:In depth knowledge and experience with income security programs (AISH, Alberta Works, CPP) at community, provincial and federal levels. Advanced knowledge of the legislation and options related to decision making capacity e.g. AGTA, Personal Directive Act ,capacity assessment pathways, Protection of Persons in Care Act and Continuing Care Health Services Standards (CCHSS). Demonstrates critical thinking.
