Social Worker II - Corrections Transition Team
Your Opportunity:
We are looking for an energetic and dynamic individual with experience in patient-centered, trauma-informed, and recovery-based care to fill the position of a Social Worker ll and join our multi-disciplinary Mental Health and Addiction Outreach team in Medicine Hat. Working both independently and as a member of a collaborative multidisciplinary team, you will provide evidence informed and best practices in crisis intervention, assessment, consultation, referral, case management and treatment planning and implementation to clients transitioning from corrections to community services. This position will focus primarily on Corrections Transition Team but may also support other Mental Health and Addiction Outreach Programs/Services as required. The Social Worker II works in an integrated and collaborative way to provide treatment for clients with addiction and mental health needs who may have been involved with the criminal justice system. As a member of a multi-disciplinary team, you work confidently, independently, and within collaborative partnerships. You have knowledge of the criminal justice system, mental health and addiction services, resources within Recovery Alberta and community stakeholders in the South Zone. Your excellent interpersonal skills support your ability to work effectively across a continuum of services. You can balance numerous demands and conflicting priorities, accurately collect, and document information in a timely manner. The successful candidate will be an effective communicator with strong organizational skills and have a demonstrated ability and comfort with decision making responsibilities, and the ability to create innovative solutions for complex and diverse issues.

Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).
Additional Required Qualifications:Previous experience in a Community Addiction and Mental Health Setting performing addiction and mental health assessments and provision of psychotherapy required. Relevant training in assessment and treatment of addiction and mental health issues is required. Knowledge of the resources within Mental Health and Addiction services & experience with clients with complex addiction & mental health issues. Preferred Qualifications:Experience in working collaboratively with other health professionals, physicians, psychiatrists, and community agencies. Previous outreach experience preferred. Previous experience working within the criminal justice system preferred. NVCI training, CPR and first aid training and suicide intervention training preferred. Experience using connect care preferred.
