Your Opportunity:

AMH Supported Housing is looking for an experienced Social Worker to join our team as a Case Manager in one of our housing programs. This role supports a 9 bed transitional housing program (WORTHII) that supports high intensity, complex clients. The Social Worker will work as part of a multi disciplinary team, in addition to working with the housing operator. Relationship building skills and the ability to work collaboratively are crucial to the role. The incumbent will be responsible for collaborative intake and the provision of clinical support and treatment planning. They will work with the housing provider and additional care teams to assist clients in their transition to long term housing. Essential to the skill set of this position will be a comprehensive awareness of housing resources, and an understanding of personal and systemic housing barriers. This position requires skill in AMH competencies such as risk assessment, mental health/ status assessment, biopsychosocial assessment. This is a fast-paced role and an ability to manage competing priorities while building a relationship with partners is critical. This position has transitioned to Recovery Alberta effective September 1, 2024. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Required Qualifications:

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).

Additional Required Qualifications:

Current CPR (BLS Level C – Health Care Provider). NCI - Non Violent Crisis Intervention. Cultural competence and a commitment to ongoing reconciliation.

Preferred Qualifications:

3-5 years experience in a community mental health and/or rehabilitation setting preferred. Flexibility, openness, and a desire to be a strong team player. Excellent communication and self-management skills. Knowledge of Concurrent Capable practice. Knowledge of both Recovery Oriented Care and Harm Reduction principles. Experience working with multiple stakeholders is an asset.
