At Cancer Care Alberta (CCA), Supportive Care services are delivered by a dedicated team of health care professionals committed to improving the quality of life for individuals affected by cancer across Alberta. This support begins at diagnosis and continues through treatment and beyond. CCA's team are recognized leaders in cancer prevention, diagnosis, treatment, survivorship, and palliative care. We are looking for a passionate Social Worker (SW II) to join our team at the Jack Ady Cancer Centre. In this part-time role, you will play a key part in supporting patients and caregivers affected by cancer by connecting them with resources and advocating for their well-being. Key Responsibilities in this role include: Referral triage, screening, intake, resource counseling, supportive counseling Working collaboratively with a team of healthcare professionals to enhance the patient experience Developing and facilitating psychoeducational groups Participating in quality improvement initiatives, research, and service evaluation. Engaging in continuous professional development and education. Supporting orientation and mentorship of new employees and students. Contributing to a collaborative team environment with excellent time management and communication skills. Connecting patients to non-AHS programs such as Wellspring Alberta. Caseloads for all positions within Supportive Care may change depending on operational needs. This role may also provide virtual or in-person coverage at the Margery E. Yuill Cancer Centre as required.

As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).
Additional Required Qualifications:Basic Cardiac Life Support-Healthcare Provider (BCLS-HCP) through Heart & Stroke Foundation. Clinical work and/or practicum in an adult cancer care setting, palliative care, hospital setting, home care or outpatient clinics. Experience providing virtual care. In-depth knowledge and experience of advance care planning, income security programs, housing, and support services. Experience providing supportive counseling, grief, and loss support. Demonstrated skills in assessment, intervention, system navigation and case management. Effective time management, self-direction and initiative are required. Excellent communication skills.
Preferred Qualifications:Proficient with electronic medical systems (e.g.Connect Care) and Microsoft Suite (e.g., Excel, PowerPoint, Word, Outlook) Within the last year, experience working in Cancer Care Alberta and/ or providing support in an adult oncology acute care setting. Experience developing partnerships / collaborations within and across organizations. Experience facilitating groups.
