The Acute Care Addiction Recovery Program (ACARP – formerly known as ARCH) team is a consult service located at the Peter Lougheed Centre, Foothills Medical Centre, and Rockyview General Hospital, specializing in addiction medicine. This position is located at RGH. The ACARP team is multi-disciplinary and comprised of medical and allied health, health care providers. The ACARP team provides wrap around services to patient(s) who present with active substance use concerns. Specializing in supporting the primary care team to offer a full spectrum of services through a recovery orientated model of care, inclusive of harm reduction, low-threshold services, trauma informed care, and offering interventions to improve social determinants of health. ACARP is a grant-based program helping patients start or progress on their recovery journeys through timely identification of those in need of supports, and by offering immediate access to a recovery-oriented system of care (ROSC). Reporting to the manager of ACARP, the SWII will work with patients who have been consulted to ACARP, in collaboration with the wider team to provide social stabilization supports. Will work to full scope as outlined in the provincial job description and the role will primarily consist of performing assessments/screening, case conceptualization, and completing a broad range of resource-based interventions to maximize social determinants of health, as well as motivation interviewing. The role supports with connecting patients to community resources via warm handoffs. This position has transitioned to Recovery Alberta effective September 1, 2024. As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).
Additional Required Qualifications:Minimum 1 year experience working in an acute care or community setting. Minimum 1 year working with clients or patients who experience substance use disorders and/or houselessness. Demonstrated knowledge of Harm Reduction principles. Demonstrated knowledge of Calgary Zone community resources. Demonstrated strong assessment and intervention skills. Demonstrated time management skills with the ability to organize and prioritize competing demands. Strong interpersonal verbal and written communication skills. Demonstrated effective collaboration within a team environment. The role will also provide education, mentorship and supervision support and participate in quality improvement initiatives, as required.
Preferred Qualifications:Recent experience in acute care. Recent experience working in addiction medicine. Recent experience working in a consult model. Recent experience working with community agencies for housing, income and identification purposes, and with marginalized populations. Ability to form relationships with community partners for health and social needs. Demonstrated knowledge of Alberta Government social supports application processes. Current BCLS – HCP, NVCI, and Connect Care Training.
