Your Opportunity:
Addiction & Mental Health Supported Housing (AMH Housing) is a fast paced and growing area within the AMH portfolio. The Flow Team is a 5-person team that focuses on improving the patient flow out of hospitals into the AMH community housing beds. They work with inpatient social workers and teams to assist with discharge planning, referral management and complex case reviews. They act as a liaison for the inpatient teams back out into the community to ensure housing placements are done as seamlessly as possible. This is a specialized position that requires a high level of knowledge of all the resources across the broader housing sector. Essential to the skillset is a comprehensive awareness of personal and systemic housing barriers. The incumbent will have a thorough knowledge of concurrent practice and will be able to demonstrate this knowledge through case presentations back to the team and housing operators. The Flow team will also field calls from the community as well as participate in complex case reviews from our internal partners as needed. This is a fast paced, high demand position and requires an ability to mediate high pressure situations, as well as an ability to think creatively when looking at housing options. Strong boundaries and time management skills are critical to success. The position involves time spent in the hospitals, meeting with both clients and inpatient teams, and thus travel throughout the Edmonton zone is required. This position has transitioned to Recovery Alberta effective September 1, 2024. As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).
Additional Required Qualifications:Current CPR (BLS Level C – Health Care Provider). Current NCI (Non-Violent Crisis Intervention). A driver's license and access to a vehicle with appropriate business insurance. 3-5 years' experience in a community mental health setting. Cultural competence and a commitment to ongoing reconciliation. Flexibility, openness, and a desire to be a strong team player.
Preferred Qualifications:Excellent communication and self-management skills. Knowledge of concurrent capable practice and recovery-oriented care. Experience working with multiple partners is an asset. Extensive knowledge of the housing sector.
