Social Worker II
Your Opportunity:
The Social Worker II works independently to provide generalist social work services to manage a complex caseload of patients admitted to Halvar Jonson Center for Brain Injury. The focus of this position is to resolve outstanding social, legal and/or financial barriers for patients transitioning to the community or continuing care following rehabilitation after an acquired brain injury. The SW II collaborates with multi-disciplinary team members, public sector and community agencies to advance care for clients. Key responsibilities include Assessment: performing screening and assessment within the context of supportive living options, as well as home environment. Assessing patients for eligibility to various social programs, benefits and resources. Case conceptualization: develops an understanding of the patient and their behavior in relation to the community/supportive living context. Intervention: case management involving the coordination of providers, services and resources to enhance integration of services and care for patients and families. Develops collaborative partnerships to facilitate access to services. Applies knowledge of relevant legislation to direct practice and assists members of the healthcare team in delivering services within legislative requirements (e.g. Mental Health Act, Adult Guardianship and Trustee Act, Personal Directives Act etc.). Mediates and supports resolution of conflicts within the family system and between the patient/family and healthcare team. Facilitates risk reduced and timely care for transitions/discharge plans. Facilitates problem-solving process with patients and families when decision making issues are identified.

As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).
