Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).
Additional Required Qualifications:This position is a sedentary position and requires long periods of keyboarding and computer use. May be keyboarding up to 6 hour a day. Due to the nature of the work, long periods of time will be spent in a seated position. Two years of recent experience preferably in a Community Health and Acute Care settings. Experience working with geriatric, mental health or complex care adult populations. Previous experience working with an interdisciplinary team. Current successful completion of Healthcare Provider CPR program is required.
Preferred Qualifications:Minimum 3-8 years’ experience in Nursing in Mental Health, Home Care, Geriatrics or Transition Services. Equivalencies with strong adult medical/surgical clinical background will be considered. Community Experience (home care or transition services). RAI certified. PARIS, SCM, Pathways experience.
