Required Qualifications:

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).

Additional Required Qualifications:

Basic Cardiac Life Support-Healthcare Provider (BCLS-HCP) through Heart & Stroke Foundation. A minimum of 2 years post-degree (Social Work) experience. Demonstrated experience working with patients who have a chronic illness. In-depth knowledge and experience of advance care planning, income security programs, housing, and support services. Demonstrated skills in assessment, intervention, system navigation and case management. Demonstrated dedication to professional development.

Preferred Qualifications:

Experience working within a chronic disease management framework with adult populations in an outpatient setting within the last 5 years. Experience facilitating group work. Experience providing education to patients and families about chronic illness management. Experience with and knowledge of community & financial resources, grief, and loss, & supportive counselling. Experience working in an adult acute care setting within the last 5 years.
