Edmonton Zone Continuing Care provides services through five main program areas: Home Living, Supportive Living, Facility Living, Transition Services and Palliative Care. We are looking for a Social Worker I to join our Supportive Living team to provide care and support to adults and seniors residing in congregate living settings. Supportive Living is a non-institutional alternative for those who are no longer able to live in their own home, which focuses on health promotion, well-being, independence and maintaining functional abilities. As a Social Worker I in Supportive Living, you will work with clients and the Specialty Services Team of professionals under the direction of the Social Worker II. Key responsibilities and competencies include: Gathering and documenting basic client information including social and economic information. Providing basic client support. Liaise with and refer clients to appropriate government and community agencies. Liaises with contracted service providers and other members of the care team. Requires knowledge of resources available to clients. Requires excellent interpersonal skills, and strong written and verbal communication skills. Requires a client-centered team approach to care.
Description:
As a Social Worker I, you will be a member of a multi- disciplinary team that provides mental health services to individuals, families and/or groups at a community based facility. You will be responsible for the initial screening of patients by gathering and documenting client information, basic assessment and providing support to clients and their families' short-term needs. You will liaise with and refer clients to appropriate community and government services and support, along with participating in case conferences and team meetings.

Completion of diploma in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).
Additional Required Qualifications:Basic Cardiac Life Support- Healthcare Provider (BCLS HCP). Communication skills to work cooperatively and effectively with the healthcare team members, clients, and their families in a fast-paced environment. Responsible to maintain active registration with the Alberta College of Social Workers (ACSW) throughout the duration of employment. The successful candidate will work independently in Supportive Living Facilities and in the community. Therefore, the successful candidate must be able to work in different settings, environments and weather conditions, as well as be able to lift and carry medical bags, which weigh approximately 25-30 lbs.
Preferred Qualifications:Education or training in mental health or geriatrics is an asset. Meditech and Connect Care experience. Recent experience working in the community with older adults who have complex needs.
