Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College and Association of Registered Nurses of Alberta (CARNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).
Additional Required Qualifications:3 to 5 years professional experience that includes both home care and acute care nursing. Experience navigating the healthcare system and community resources. Negotiation & conflict resolution skills. Knowledge of Advanced Care Planning & legal decision making.
Preferred Qualifications:Experience with transition services and discharge planning; Knowledge of computer applications (Netcare, Meditech, MS Office). Experience with RAICA & RAIHC. AHS McMaster Case Management. Knowledge of Collaborative Care & experience working with interdisciplinary teams. Ability to advocate for clients and families. Baccalaureate degree in nursing preferred; however, skills and experience relevant to nursing practice in community sector will be considered.
