Edmonton Zone Continuing Care provides services through five main program areas: Home Living, Supportive Living, Facility Living, Transition Services and Palliative Care. We are looking for a Registered Nurse to join our Continuing Care Access team to provide professional support services for clients of all ages and stages of life. If you are interested in playing a key role in linking individuals in the community with programs and services both internal and external to Alberta Health Services, then this position may be right for you. Using strong tele-triage skills, you will assess, identify and clarify the needs of the client and triage referrals to appropriate agencies and service partners. You will provide after-hour telephone support to current Continuing Care clients and monitor the health status of clients in the community, all within a fast paced work environment where team work is valued and professional development is encouraged.
Description:
As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values of AHS. PLEASE NOTE: Provisional Permit Holders shall be paid $39.58 /hour - $54.16/hour. Upon being entered into the register of Registered Nurses, the rate of pay shall be adjusted to the rate of pay applicable to a Registered Nurse in accordance with AHS/UNA Article 25.02(b)(i) or (ii) as applicable.

Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP). If you have not practiced as a Registered Nurse within the last five (5) years, completion of the following free courses is required: CRNA Medication Management - Chapters 1 - 4 (https://connect.nurses.ab.ca/home/%2Flearning-and-development%2Flearning-modules%2F) and the NextGenU Health Assessment Resources Certification (https://courses.nextgenu.org/course/view.php?id=271#section-0)
Additional Required Qualifications:Minimum of 1-2 years of recent Continuing Care community nursing (i.e. Home Living, Supportive Living) experience is required. Excellent verbal, written and telephone communication skills are necessary, as well as experience tele-triaging complex clients and situations. Knowledge and use of Connect Care systems is essential/mandatory Responsible to provide ongoing proof of current CPR, educational credentials and active registration which is in good standing with the College of Registered Nurses of Alberta (CRNA). It is also the employees’ responsibility to maintain registration throughout the duration of employment. At the time of interview, you may be required to demonstrate your computer knowledge and proficiency by completing a hands-on assessment. You must demonstrate sufficient typing speed to meet workload demand and navigate through various computer programs including Microsoft Office (Word, Excel, Access, PowerPoint), Visio, Outlook and Internet. Physically capable of performing nursing tasks including extended periods of repetitive motion tasks.
Preferred Qualifications:Baccalaureate degree in nursing preferred; however, skills and experience relevant to nursing practice in community sector will be considered. Palliative care experience. Previous Continuing Care and/or geriatric experience. Experience with Connect Care and the RAI Home Care assessment tool. 2 years current Case Management Experience in Continuing Care. Experience with advanced telecommunications systems would also be a benefit.
