Registered Nurse
Your Opportunity:
Reporting to the Care Manager on the Seniors South Team within Integrated Home Care, the RN Case Manager/Interventionist works as a member of an inter-professional health care team. Within the full scope of the professional practice, is responsible for assessing, planning, organizing, reviewing and evaluating the needs of Acute and Rehabilitation clients and their caregivers/families requiring community based services that fosters and promotes continuity of care and co-operative partnerships by liaising with primary care physicians, residential care facilities, acute care hospitals, and other programs/organizations involving provision of services and may work in multiple settings. The RN Interventionist/Case Manager (for Acute/Rehabilitation client groups) is also responsible for providing direct nursing interventions to a select group of Long Term Supportive, Maintenance and End-of-life clients in a designated area on the Seniors South Team-Integrated Home Care Program.

As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values. PLEASE NOTE: Provisional Permit holders will be paid $40.76 to $55.79 per hour. Upon obtaining a full practice permit, the rate of pay will be adjusted to the applicable Registered Nurse rate in accordance with UNA Article 25.02(b)(i) or (ii), as applicable.

Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP) certification. Additional Required Qualifications:This is an ON-SITE position. You will be required to start and end your day at the office. Client home visits in the community are required each day. There is no opportunity to work from home. Minimum two years of RN community nursing experience required. Excellent interpersonal, communication, and time management skills with a strong ability to work in a collaborative team demonstrating adaptability and flexibility. Proven critical thinking, decision-making, and assessment skills. Physically able to perform all assigned tasks including manual handling (lifting, lowering, carrying, pushing and pulling) as a regular part of the job. For example: lifting of approximately 30 pounds in and out of a vehicle on a regular basis and carrying this weight up and down stairs throughout the day is common within this job. Ability to perform all duties in accordance with related legislation.
Preferred Qualifications:Baccalaureate degree in nursing preferred; however, skills and experience relevant to nursing practice in community sector will be considered. Gerontology certificate is an asset. Experience with Connect Care. Proficiency with Microsoft Office. Proficiency with a laptop and mobile phones application.
