Your Opportunity:
Reporting to the Care Manager and/or designate, this position provides a consultative, non-emergent, specialized assessment for the senior population requiring mental health services. The incumbent will provide education and information to individuals/families. As determined by Service Mandate, interventions will be provided in a variety of community settings Wetaskiwin and surrounding area. The nurse addresses the complex needs of the elderly persons in the community, taking into account the aging process, with a goal to maintain individuals in their residential environments. Duties include: Maintain relevant client data as per policy/standards, including screening for risk. Negotiates the treatment plan with the client/family/caregivers and recommends interventions that assist the clients to maintain optimal levels of functioning in their current environment. Responding, screening, and prioritizing referrals from the community (individuals, professionals, and agencies). Conduct the necessary assessments and observations to observe appropriateness and referral recommendations to alternate programs, agencies, and/or other resources. Providing assessments to identify the presence of psychiatric/cognitive/functional deficits and need for services. Assessing data and arriving at a diagnostic impression in consultation with other disciplines as needed. Monitoring and assessing mental status and client functioning to avoid psychiatric admission or re-admission. Providing consultation to other clinical staff and/or community agencies. Supporting families/care providers in maintaining clients in a community setting. Provides follow-up service on referral from Centennial Center for Mental Health and Brain Injury (CCMHBI) and other facilities to assist in the implementation of treatment suggestions and reintegration into the community.

As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values of AHS. PLEASE NOTE: Provisional Permit Holders shall be paid $39.58 /hour - $54.16/hour. Upon being entered into the register of Registered Nurses, the rate of pay shall be adjusted to the rate of pay applicable to a Registered Nurse in accordance with AHS/UNA Article 25.02(b)(i) or (ii) as applicable.

Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP). If you have not practiced as a Registered Nurse within the last five (5) years, completion of the following free courses is required: CRNA Medication Management - Chapters 1 - 4 (https://connect.nurses.ab.ca/home/%2Flearning-and-development%2Flearning-modules%2F) and the NextGenU Health Assessment Resources Certification (https://courses.nextgenu.org/course/view.php?id=271#section-0)
Additional Required Qualifications:Minimum 3 years mental health experience in the past ten years, with 2 years’ experience working with Seniors. Training in Connect Care. Knowledge on designated mental health facilities, community resources, and certification process. This position has transitioned to Recovery Alberta effective September 1, 2024.
