Baccalaureate Degree in Nursing (a combination of experience and education may be considered). Active registration and practice permit with CARNA. Additional clinical courses or certification in specialized palliative and end of life practice area Minimum of 10 years experience in nursing with 5 years recent experience in palliative care specialty area. Experience with Quality Improvement processes and tools. Current Basic Cardiac Life Support – Health Care Provider (BCLS-HCP).
Additional Required Qualifications:
Strong background in working with patient centered model of care. Extensive knowledge of rural clinical operations. Ability to lead functional and project teams. Experience with virtual teams an asset . The Practice Lead applies a depth and breadth of specialized knowledge to support the Palliative Care program to achieve the goals of quality, access and sustainability in the area of Palliative Care. Excellent leadership, communication and interpersonal skills to establish and maintain trusting relationships with internal and external partners. Demonstrated ability and comfort with decision making, coaching and mentoring. Strong analytical skills with the ability to problem solve utilizing current resources. Committed role model for the AHS values of respect, accountability, transparency, engagement, safety, learning and performance. Ability to be work independently, multitask and set priorities.
Preferred Qualifications:
Master’s Degree an asset. CNA Certified in Hospice and Palliative Nursing – CHPCN (c ).
