Your Opportunity:

The Pharmacist I will provide direct patient care in accordance with the AHS Pharmacy Services clinical expectations standards at the University Hospital, Stollery Childrenʼs Hospital or Mazankowski Alberta Health Institute. This includes managing medication therapy for AHS patients collaboratively with other healthcare providers in a patient centered model to ensure that medications are used safely, effectively and appropriately. Both initially and on an ongoing basis, you will assess patients for potential or actual drug-related problems and resolve or prevent drug-related problems in order to optimize patient outcomes. Actively participating in patient care rounds where applicable. Liaising with members of the health care team and developing a professional relationship with the patient and their family to formulate a medication treatment plan. Documenting all direct patient care activities in the patientʼs Health Record, participating in organizational practices, drug stewardship, and quality improvement initiatives to improve quality of patient care. Obtaining and/or incorporating additional prescribing authorization wherever practice allows and participate in the collection of key performance indicator data and clinical workload measurements. Maintaining their own professional development and participating in the precepting and mentoring of other staff and students. All clinically deployed pharmacists will have regular dispensary duties including medication order assessment and verification during dispensary shifts. Pharmacist may be checking prepared oral, parenteral, chemotherapeutic and topical medications., dispensing and coordinating the procurement of special access and study medications, providing functional supervision for pharmacy operations and answering drug information questions directed to the dispensary.

Required Qualifications:

Bachelor of Science in Pharmacy. Active or eligible for registration with the Alberta College of Pharmacy (ACP).

Additional Required Qualifications:

Ability to use computer medication order entry systems and able to access and proficiently use drug information resources and word processing. Fluency in written and verbal English language. Ability to incorporate AHS organizational values into daily work, actions and decisions. Ability to perform duties as outlined on the Physical Demand Statement (PDS) for the position. Ability to safeguard AHS information as private and confidential. Ability to follow and adhere to AHS policies, procedures, goals and objectives. Ability to communicate effectively both verbally and in writing. Proven initiative and motivation is required. Ability to work with a wide variety of people in health care environment with professionalism and respect. Ability to effectively manage his or her time and setting priorities when faced with multiple tasks and deadlines. Ability to provide a positive and friendly approach that will enable you to deal with challenging situations with empathy and sincerity. Demonstrated problem solving skills. Must demonstrate personal and professional accountability.

Preferred Qualifications:

Experience with medication dispensing cabinets/unit dose drug distribution preferred. Additional Prescribing Authorization and to administer injections preferred. PRAC ID preferred. IV additive knowledge/TPN knowledge/ Chemotherapy certification preferred. Completion of an accredited hospital residency/Pharm D program preferred. Large Acute Care hospital experience delivering proactive, integrated clinical practice is preferred (at least 1 year within the last 3 years).
