Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP). Additional Required Qualifications:Project management experience or expertise. Quality improvement experience. Demonstrated computer and electronic health record documentation skills required. Minimum of 3 years of recent or current nephrology and/or chronic disease management experience required. Ability to maintain effective and respectful working relationships required. Experience in the Case Management process of care with in the last 5 years. Demonstrated excellent interpersonal, organizational, assessment, and teamwork skills required. Ability to apply the principles of adult learning required. Experience and/or specialized training in diabetes, hypertension, cholesterol, and/or adult modality education principles. Experience in policy development and patient education materials.
Preferred Qualifications:Baccalaureate Degree in nursing or other related degree. Canadian Nurses Association (CNA) in Nephrology Nursing. Experience as a RN in a chronic disease management setting within the last 5 years. Experience in leading group education for patients and staff. Experience in leading multidisciplinary rounds. Experience in data collection and analysis. Experience working with various stakeholders outside of Alberta Kidney Care South. Experience working with Indigenous populations.
