Required Qualifications:

Completion of an accredited Licensed Practical Nurse education program. Active or eligible for registration and practice permit with the College of Licensed Practical Nurses of Alberta (CLPNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).

Additional Required Qualifications:

Successful completion of Certification in Immunization required; experience in administering immunizations. Experience working with complex high risk clients. Experience in working with addictions and/or mental health. Recent skills with wound care assessment and management required. Experience with an EMR system and demonstrated aptitude with computers and technology required. Demonstrated attention to detail and accuracy. Ability to be self-motivated and work independently. Strong communication skills utilizing a variety of modalities and ability to adapt communication style to meet the needs of a specific patient; relay patient information to other health care providers in a timely, concise, accurate manner in order to facilitate the primary health care providers’ treatment decisions. Assessment of skills to recognize and respond appropriately to changes in patients’ status. Balance numerous demands and conflicting priorities. Work within a team environment and effectively contribute to patient planning processes.

Preferred Qualifications:

Three years recent ambulatory and outpatient experience. Primary Care experience. Experience in providing diabetic foot exams. Experience in providing education and guidance to unregulated health care workers preferred. Experience in case management.
