Edmonton Zone Continuing Care provides services through five main program areas: Home Living, Supportive Living, Facility Living, Transition Services and Palliative Care. The Case Management Practice Lead provides leadership in clinical practice and client care. Enhances client care outcomes and staff professional development through expert clinical practice, consultation, research and education. Responsible to assess, create, facilitate, coordinate and evaluate the implementation of education to promote clinical competence. Functions as a leader, innovator and change agent in the pursuit of excellent in client care. Supports new staff and promotes best practice related to case management. Participates in Client Reviews/SMC reviews and encourages a person-centered focus and outcomes. Supports the provincial role out of the McMaster education in Case Management. Supports the use of RAI as a clinical assessment tool through the provision of RAI reviews, and one-on-one coaching and mentorship. Assists with change management and system transformation initiatives. Supports clinical service delivery including the integration of Connect Care into case management. Supports staff in complex client situations/system navigation. Attends home visits, case & discharge planning conferences, with Case Managers. Chairs clinical staff meetings and committee meetings. Maintains competence in advanced clinical practice relevant to Home Living and acts as a consultant to staff/management regarding clinical issues. Influences and supports a culture of professional growth. Assists with the assessment of education needs to staff and the identification of appropriate resources.
Description:
As an Instructor, you will be an expert practitioner and key educator. You will take a leadership role in the planning, coordination, implementation and evaluation of education for nursing staff including orientation, compulsory and continuing education, and clinical development. You will collaborate with management and internal partners to promote standards of clinical, educational and evidence informed practice. You will play a key role in providing safe, quality patient and family centered care while reflecting the shared vision and values of AHS.

Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).
Additional Required Qualifications:Baccalaureate Degree in a health discipline, education, and/or leadership/administration. Minimum of 5 years of recent Home Living experience. Experience with development, implementation, facilitation and evaluation of education materials and programming is required, within the Home Living portfolio. Basic Cardiac Life Support – Healthcare Provider (BCLS-HCP) is required. Ability to evaluate clinical performance, assess staff clinical needs and plan appropriate education strategies. Demonstrated leadership and facilitation skills. Strong collaborative and interpersonal skills as well as excellence in written and verbal communication. Coaches others and promotes teamwork and partnerships to attain optimal client outcomes. Recognized for clinical expertise in Home Living Program areas. Demonstrates creativity, flexibility and initiative. Displays highly developed organizational skills, critical thinking skills and problem-solving ability. The successful candidate will work in different settings and environments (home environments and weather conditions), as well as be able to lift and carry supply bags, which weigh approximately 25-30 lbs. At the time of interview, you will be required to demonstrate your computer knowledge and proficiency by completing a skills test. Must have working knowledge of computer programs including Microsoft Office (Word, Excel, Access, PowerPoint), Visio, Outlook and Internet
Preferred Qualifications:Post graduate studies or certification in adult education, gerontology, chronic disease management and/or palliative care would be an asset. Masters degree in a health related field is an asset. Participation in research activities is an asset. Management experience is an asset. Project management is an asset.
