Consultant - Senior Consultant - Continuing Care Programs

Alberta Health Services (AHS) needs someone like you to provide direction, team leadership, and organization. Although this full-time role does not have any direct reports, you would support, mentor, and collaborate with several colleagues and professionals within the senior’s health portfolio. The Senior Consultant of Continuing Care programs for Central Zone NE, collaborates with Area Leaders, Central Zone programs along with direct care providers to ensure that clients receive quality care in a safe competent and ethical manner. This role is pivotal to ensure implementation of all continuing care programs are implemented in a consistent manner with clear sight for program sustainability while ensuring all facilities have implemented clinical best practice guidelines and standards of care with an emphasis to align rural settings. This position will require working with a matrix of Continuing Care Directors who have programmatic responsibilities to ensure consistency and continuing of implementation of programs within the Central zone facility based Continuing Care, Home Care, Designated Supportive Living, PEOLC and ADSP. This position will support multiple rural sites such as Lloydminster, Vermilion, Mannville, Viking, Galahad, Tofield, Two Hills, Hardisty, Lamont, Vegreville and Wainwright. Please note this role has flexibly on primary location and travel to the variety of sites would be a requirement.
Description:
The Senior Consultant of Continuing Care collaborates with Area Leaders, Central Zone programs and direct care providers to ensure that clients receive quality care in a safe, competent, and ethical manner. This position will liaison with central zone departments, site and medical staff, external organizations, and communities in order to coordinate and integrate client programs and services within the Long-Term Care Program, Home Care Program, Supportive Living Program, Adult Day Support Program, Palliative End of Life Program, and the Home parenteral Program. The Continuing Care Program Manager provides leadership to several Continuing Care programs in keeping with the policies and procedures outlined by Central Zone, Alberta Health Services and Alberta Health. This position is Directly accountable to the Continuing Care Director Central Zone Northeast.

Post-secondary degree / diploma in a health profession, health administration, leadership, or other comparable degree / diploma. A demonstrated clear pattern of professional development (e.g., continuing education courses). Minimum 5 years' experience with Continuing Care.
Additional Required Qualifications:As Required.
Preferred Qualifications:Clear history of Continuing Care experience including Home Care / Supportive Living / Long Term Care. Previous experience with Managing a unionized workforce.
