Required Qualifications:

Knowledge of the Urban Palliative Home Care program and its interconnection with other health systems and programs. Knowledge of person-centered care approach to service delivery. Knowledge of internal and external resources relevant to the delivery of Palliative Home Care. Working knowledge of Microsoft Word, Excel, PowerPoint, and Outlook. Excellent time management and organizational skills. Strong interpersonal and communication skills. Effective conflict resolution and proven leadership skills. Ability to manage competing priorities effectively. Effective analytical and problem-solving skills. Demonstrated ability and comfort with decision making responsibilities. Computer literacy in Microsoft Office (Word, Outlook), ePeople/Recruitment Management System, and Environment for Scheduling Personnel (ESP) is required. Staff Development: Fosters an atmosphere conducive to encouraging and meeting staff development needs and student learning experiences. Attendance and Performance Management: Monitors and documents staff attendance and performance. Provides immediate and constructive feedback while working with the staff to facilitate ongoing quality performance.


Additional Required Qualifications:

Recruitment and Orientation: Initiates the recruitment process and completes the hiring process. Works collaboratively with other Managers for recruitment, interviews and casual staffing. Facilitates orientation of newly hired or returning staff and supports ongoing staff orientation and educational needs. Minimum Bachelor degree in health related field required; Masters preferred. An equivalent combination of experience and education will be considered. A minimum of 5 years of clinical practice or health related experience required in community palliative care (current community care preferred).


Preferred Qualifications:

Previous management experience in a health related field is an asset; experience in Continuing Care preferred. Knowledge of community resources is an asset.
