Required Qualifications:

Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP). If you have not practiced as a Registered Nurse within the last five (5) years, completion of the following free courses is required: CRNA Medication Management - Chapters 1 - 4 (https://connect.nurses.ab.ca/home/%2Flearning-and-development%2Flearning-modules%2F) and the NextGenU Health Assessment Resources Certification (https://courses.nextgenu.org/course/view.php?id=271#section-0)

Additional Required Qualifications:

Minimum 3 years nursing experience required. Previous nursing practice in community and in the geriatric sector preferred. Knowledgeable with programs PARIS, Strata pathways. In addition, the incumbent will be expected to demonstrate proficiency in the following areas: Demonstrated knowledge of community agencies and resources in order to facilitate the linkage to appropriate services. Demonstrated conflict resolution, problem-solving, negotiation and facilitation skills. Excellent organizational and time management skills to efficiently organize and prioritize workload. Ability to be self-directed and exercises independent decision making, nursing judgment and critical thinking skills. Excellent interpersonal and communication skills to effectively work with a multi-disciplinary team of Physicians, Nurses and Allied Health professionals. Actively encourages and integrates client and family centered care principles within the nursing team and own nursing practice. Ability to provide comprehensive assessment skills of individuals within the context of their physical and social environment. Ability to monitor financial accountabilities related to care delivery. Shown commitment to maintaining knowledge and skills through professional development or continuing education. Excellent written, verbal and computer processing skills to appropriately capture pertinent client information.

Preferred Qualifications:

Minimum 3 years community experience (IHC, ISFL) and or current/previous Transition Services Coordinator. Strong conflict resolution and change management skills are preferred. Strong computer skills and ability to navigate and data entry into a variety of software programs some of which include PARIS, Strata Health programs, Sunrise Clinical Manager and upcoming Epic -connect care.
