Required Qualifications:

Completion of post-secondary diploma in a related field.

Additional Required Qualifications:

5 years of experience in Continuing Care. Minimum 1 year of practice experience using Connect Care/Epic resources, with organizational and/or clinical workflows, and exposure to the launch readiness approach. Advanced computer skills in MS Office and in clinical systems such as EPIC, Meditech, Pathways, HomeFirst. Prior work experience facilitating, assisting and or supporting colleagues in a professional environment. Experience in educational material development and training delivery. Demonstrated ability and comfort with decision-making, coaching and teaching is required. Strong collaborative and interpersonal skills. Excellent in written and verbal communication. Ability to work with a significant degree of independence. Highly developed organizational skills, critical thinking skills and problem-solving abilities. Proven ability to prioritize and organize workload and multiple priorities and meet deadlines.

Preferred Qualifications:

Additional experience with other software such as MS Access, Visio, Adobe Writer and Adobe Captivate is an asset. Effective change agent; change management training or experience. Experience related to the implementation and support of software solutions.
