Social Worker III-Lead Clinician Frequent Visitor to Emergency Program
Your Opportunity:
Are you looking for an opportunity to grow your Social Work (SW) training and experience while making a real impact on the lives of Albertans? The Frequent Visitor to Emergency Department Program and Royal Alexandra hospital Social Work Department is currently searching for a Social Worker III to join our team. This position will provide SW input and guidance for an interdisciplinary team to improve care for patients who present frequently to the Emergency Department (ED) with complex medical, mental health and social needs. This role will champion coordinated, patient-centred approaches that reduce repeat presentations, improve outcomes, and strengthen integration between acute, community, and primary care sectors. The position will involve working as a collaborative interdisciplinary team to create individualized care plans aligning with evidence informed and best practice strategies and to identify those patients whose complex needs would be well served by an individualized care plan and standardized care on their ED visits. This position will provide clinical consultation on complex or high risk cases and build strong relationships with stakeholders outside of the ED and AHS (primary care, home care, recovery Alberta, local shelters, housing providers etc.).
