Your Opportunity:

The Community Accessible Rehabilitation (CAR) program is an ambulatory rehabilitation program for clients with complex neurological and musculoskeletal conditions. This advanced level clinical social work position works with a collaborative interdisciplinary rehabilitation team serving an ambulatory adult population in the Community Accessible Rehabilitation (CAR) Program. The successful applicant will be responsible to provide Social Work assessment, treatment, and consultation services for the CAR clients and ensures that social work clinical practice is evidence based and meets professional standards of practice. The position involves provision of direct patient care, consultation to CAR staff and clients, orientation of new staff and students to the service, participation in staff development, and participation in CAR program development and evaluation. The successful applicant should demonstrate initiative, leadership qualities and a patient-centered collaborative approach. The successful candidate will have a strong working knowledge of community and financial resources and supports, and be able to provide psychosocial counselling to clients to help them overcome barriers and optimize their rehabilitation potential. CAR is a multisite program and this position is based at the Peter Lougheed Center. The work schedule is 0800-1615, Tuesday and Thursday. The caseload for all positions within the CAR Program may be subject to change based on operational considerations.

Description:

As a Social Worker III, you will perform either advanced level assessment and treatment or act as a clinical lead. You will work with or within teams to provide health care services for patients with complex and multidimensional medical, psychosocial, environmental and instrumental needs. You will provide advanced clinical services, consultation to the interprofessional team and patients/families and supervision of students and team members as assigned. You may also act as a clinical lead providing leadership in the planning, development, and delivery of services in a specific area, as well as staff development, quality improvement and evaluation or research activities as identified by the needs of the service area(s).

Required Qualifications:

Master's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).

Additional Required Qualifications:

At least 5 years of clinical social work experience. At least 3 years of counselling experience. Good working knowledge of community and financial resources in the Calgary Zone. Experience working in a multi-disciplinary out-patient or community setting with adult clients. Excellent verbal and written communication skills. Demonstrated ability to function effectively within a patient-centered, interdisciplinary team of health care professionals. Demonstrated initiative and leadership abilities. Demonstrated program development and evaluation experience. Strong interpersonal and communication skills.

Preferred Qualifications:

Experience working in a rehabilitation setting. At least 1-year recent (within past 3 years) experience working with clients with complex and/or neurological diagnoses, such as stroke and brain injury. Recent experience (within past 3 years) with group facilitation. Recent experience (within past 3 years) with virtual service delivery.
