Your Opportunity:

This is a highly active clinic with a robust multidisciplinary team. The Social Worker II - Diabetes Clinic will have great organizational skills, recent acute pediatric experience and good knowledge of chronic disease impact on children and families. Adjustment disorder counselling and a patient and family centered approach is mandatory.

Description:

As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Required Qualifications:

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).

Additional Required Qualifications:

Recent acute pediatric experience. Highly organized and able to priority set is required.

Preferred Qualifications:

Knowledge of community resources essential.
