Social Worker II
Your Opportunity:
The Social Worker II – Assisted Living Alberta plays a key role in supporting patient transitions and care coordination across Acute Care, Home and Community Care, and Continuing Care Home settings. This role provides clinical social work services with a strong focus on financial assessment, resource navigation, psychosocial evaluation, counseling, case management, and discharge planning. The ideal candidate demonstrates strong clinical judgment, excellent communication skills, and the ability to work both independently and as part of a collaborative healthcare team. This role requires adaptability, critical thinking, and a commitment to patient-centered care within a fast-paced and evolving environment. This position is based out of Grande Prairie, but primarily supports the towns of Beaverlodge, Hythe and Grande Cache, with the possibility of providing services to nearby communities as needed. Responsibilities span Acute Care, Home and Community Care, and Continuing Care Homes. Travel is required, and a reliable vehicle with business insurance is necessary. Mileage or travel-related compensation is provided by the employer. In addition to a competitive rate of pay, AHS currently has a Remote Retention Allowance in place to an annual maximum of $3000. This allowance is non-pensionable and is payable on an hourly basis for all hours paid at the basic rate of pay. As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).
Additional Required Qualifications:Certification in Basic Life Support - Healthcare Provider.
Preferred Qualifications:Connect Care training. Non-Violent Crisis Intervention certification. Certification in Basic Life Support - Healthcare Provider A minimum of one year of experience in Acute Care, Continuing Care, and/or Home Care. Enhanced knowledge of community-based supports and services. Experience and knowledge related to AHS/ALA system navigation, care placement processes, transitions in care, and available financial supports.
