Your Opportunity:

Practicing at South Health Campus (SHC) means working in an environment fundamentally built on four pillars: Patient and Family Centered Care, Collaborative-Practice, Innovation, and Wellness. This position presents a unique opportunity to work with the Assisted Living Alberta Social Work division. As the Outpatient Social Worker with the Outpatient Medicine Clinics, you will be a vital interprofessional team member on the unit, but you will not be practicing in a silo. You will be joining a much larger, highly integrated social work team that seamlessly spans both acute hospital care and the broader community. In this role, you will provide specialized psychosocial care, complex resource navigation, and emotional support to patients managing acute and chronic health conditions. Moving beyond a traditional acute-care model, you will leverage collaborative practice to connect medical treatment directly to community living. By ensuring patients and their families are empowered across the continuum of care, you will help bridge the gap between hospital visits and successful community or assisted living transitions. As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Required Qualifications:

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).

Additional Required Qualifications:

Minimum of 3 to 5 years of recent clinical social work experience, preferably in a healthcare of medical outpatient setting. Demonstrated expertise in conducting comprehensive psychosocial assessments and formulating complex psychosocial care plans. Advanced knowledge of the healthcare system, including deep familiarity with Calgary-area community resources, financial support programs, and continuing care transitions. Proven ability to practice collaboratively within a high-functioning interprofessional team, actively breaking down clinical silos to optimize patient care. Strong crisis intervention skills and the ability to independently manage a dynamic, fluctuating outpatient caseload. Ability to sustain focused attention for extended periods of clinical documentation, digital charting (Connect Care), and virtual resource navigation.

Preferred Qualifications:

Direct clinical experience in ambulatory care, chronic disease management, or a specialized outpatient medicine clinic. Specific training or certification in brief counseling modalities (e.g., Cognitive Behavioral Therapy, Motivational Interviewing, or Solution-Focused Therapy). Experience in grief, loss, and adjustment-to-illness counseling. Previous involvement in continuous quality improvement initiatives, demonstrating an alignment with the SHC "Innovation" pillar.
