Social Worker II
Your Opportunity:
The Leduc Hospital is seeking a temporary Social Worker who can support emergency department and inpatient work across distinctive settings. This position requires a high degree of independent critical thinking and the ability to work collaboratively with a multidisciplinary team. You will have the opportunity to provide support to patients and families across a range of clinical activities including decision-making, finances, housing, crisis intervention, supportive counselling, resourcing, and system navigation. You will be required to prioritize workload, formulate assessments and create intervention plans while communicating closely with hospital staff. The Social Worker will demonstrate exceptional communication skills with patients, family members and hospital staff that includes appropriate and professional Connect Care documentation.
Description:
As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).
Additional Required Qualifications:Experience working in an inpatient setting contributing to discharge planning. Experience collaborating with hospital transitions and community housing providers. Experience supporting healthcare teams in the areas of capacity and decision-making. Experience working with risk and supporting patients and families to manage risk. Experience navigating ethics and ethical decision-making. Experience with supportive counseling.
