Your Opportunity:

This position is responsible for providing Clinical Social Work under the care of the Palliative Home Care Program within the context of an interdisciplinary team. This includes direct clinical care, consultation, education, program development and research. Services for the client and their families include psychosocial assessment, treatment approaches, using individual and family counseling methods, resource counseling, and service delivery evaluation. PLEASE NOTE: This is an ON-SITE position that requires reporting to an office site daily. There is no opportunity for remote work.

As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Required Qualifications:

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).

Additional Required Qualifications:

A minimum of 3 years Social Work experience in a relevant role. A minimum of 2-3 years experience as a Social Worker in a Community setting. Experience with case management. BCLS-HCP required. Palliative care experience. The ability to facilitate difficult conversations, navigate complex interdisciplinary teams, and provide culturally sensitive, holistic care. Experience and advanced skill in crisis intervention/family therapy/bereavement counselling with individuals and families facing chronic, critical and life-limiting illness. A valid driver's license and access to a motor vehicle in good repair for work related travel. Must be physically able to perform all aspects of the position in accordance with the Hazard Identification Assessment and Control (HIAC) form. This includes the ability to lift 25lbs, walk a minimum of 3km/shift, and able to tolerate bending, lifting, twisting in accordance with Move Safe training. Must exhibit a high level of self-awareness with regard to death and dying issues and demonstrate a respect for, and understanding of, individual differences in coping strategies. *Orientation to this role is full-time for 6 consecutive weeks for applicants external to Home Care. Applicants must be able to commit to this 6-week requirement.*

Preferred Qualifications:

Completion of a Master's Degree preferred. Previous community related experience. Demonstrated ability to work within a self-directed environment with minimal supervision. Ability to perform all duties in accordance with related legislation.
