Social Worker II
Your Opportunity:
The University of Alberta Hospital (UAH) offers specialized diagnostic and treatment services to adult patients, as well as specialty ambulatory clinics located across the hospital campus. This Social Worker II position is part of the site Social Work team and will be responsible for providing social work services to the adult population at the Kaye Edmonton Clinics. The candidate will be a highly motivated, confident, and dependable individual who collaborates with interdisciplinary teams, patients and their families in a busy environment ensuring trauma-informed care. The SWII will hold working knowledge of health care systems, SDoH, and community resources. They will support individuals with new diagnoses and/or living with chronic diseases through psychosocial assessments; specialized resource interventions; education; crisis intervention; adjustment counselling; advocacy; liaising with external agencies; and consultation with other care providers. As a Social Worker II you are part of a team dedicated to providing quality patient centered care through best practices and evidence informed approaches, aligning with social work Code of Ethics.
Description:
As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).
Additional Required Qualifications:Minimum 2 years' experience as an RSW in inpatient/ambulatory healthcare settings, with minimum 1 year experience supporting patients/families facing chronic and/or life-threatening illnesses required. Experience working with vulnerable populations, diverse cultural groups and new immigrants, reflecting cultural humility and awareness to unique needs is essential. The candidate will demonstrate strong case management skills with excellent organizational and time management skills to successfully manage changing priorities and workload demands. Experience with both self-direction for independently working, and with actively and positively contributing to an interdisciplinary team are essential. Up-to-date knowledge of practice standards, SDoH, and navigation of system processes to ensure advocacy and equity for patients is necessary. Able to ensure concise, timely and accurate communication, documentation and collaboration with colleagues, patients and families. Current Basic Life Support (BLS-HCP) certification is required.
Preferred Qualifications:A Master of Social Work degree (MSW) is an asset. Authorization to perform psychosocial interventions preferred. Knowledge and experience with Connect Care is preferred. Experience with relevant legislation (including Personal Directives Act, Adult Guardianship and Trusteeship Act), decision making processes, and conflict resolution are assets.
