Social Worker II
Your Opportunity:
The Social Worker II with Assisted Living Alberta plays a key role in supporting patient transitions and care coordination across care streams. This role provides clinical social work services with a strong focus on financial assessment, resource navigation, psychosocial evaluation, counseling, case management, and discharge planning. In this position, you will assist patients and families in accessing financial benefits and liaise with government agencies and community partners to ensure support is in place. You will collaborate closely with an interdisciplinary team, including physicians, nursing, allied health professionals, and external service providers, to promote safe and sustainable transitions. The ideal candidate demonstrates strong clinical judgment, excellent communication skills, and the ability to work both independently and as part of a collaborative healthcare team. This role requires adaptability, critical thinking, and a commitment to patient-centered care within a fast-paced and evolving environment. This position provides Social Work services at the Queen Elizabeth II Ambulatory Care Centre Transition Unit. The successful candidate will deliver direct Social Work support to the assigned unit and provide coverage to other areas as required. Experience with these diverse patient populations is considered an asset. Unit-specific responsibilities may evolve based on operational needs and patient demand. In addition to a competitive rate of pay, AHS currently has a Remote Retention Allowance in place to an annual maximum of $3000. This allowance is non-pensionable and is payable on an hourly basis for all hours paid at the basic rate of pay.

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).
Additional Required Qualifications:Certification in Basic Life Support - Healthcare Provider.
Preferred Qualifications:Connect Care training. Non Violent Crisis Intervention certification. Certification in Basic Life Support - Healthcare Provider. A minimum of one year of experience in Acute Care, Continuing Care, and/or Home Care. Enhanced knowledge of community based supports and services. Experience and knowledge related to AHS/ALA system navigation, care placement processes, transitions in care, and available financial supports.
