Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).
Additional Required Qualifications:Basic Cardiac Life Support-Healthcare Provider (BCLS-HCP) through Heart & Stroke Foundation. A minimum of 2 years’ post-degree (Social Work) experience. Demonstrated experience working in an urban adult cancer care setting, palliative care, inpatient and/or outpatient care in the last 2 years. Within the last two years, experience providing virtual care. In-depth knowledge and experience of advanced care planning, income security programs, housing, and support services. Demonstrated skills in assessment, intervention, system navigation, supportive counselling and case management. Demonstrated dedication to professional development. Proficient with electronic medical systems (e.g. Connect Care). Proficient with Microsoft Suite (e.g. Excel, PowerPoint, Word, Outlook). Demonstrated ability to multitask and manage competing priorities.
Preferred Qualifications:Experience working within chronic disease management framework with adults in urban outpatient settings within last 5 years. Experience in an adult acute care setting within the last 5 years. Experience facilitating groups & providing education to patients/families about cancer. Experience with and knowledge of community and financial resources as well as grief and loss. Experience developing partnerships / collaborations within and across organizations.
