Your Opportunity:

The Leduc Hospital is looking for a Social Worker motivated by a high level of independence and responsibility to support patient care in an acute setting. This position requires fast-paced critical thinking and the ability to work collaboratively within a multidisciplinary team. You will be required to prioritize referrals and workload, formulate assessments and create intervention plans in a short timeframe. These plans include psychosocial interventions, risk assessments, recommendations and provision of referrals for ongoing treatment and/or discharge planning. This position requires exceptional communication skills with patients, family members, colleagues, and physicians while demonstrating practice excellence in alignment with professional standards, ethics, regulation, legislation, and Assisted Living Alberta requirements.

Description:

As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Required Qualifications:

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).

Additional Required Qualifications:

Must have experience in an inpatient adult acute care setting. Demonstration of strong critical thinking and problem solving is required. Experience working successfully in a collaborative interdisciplinary team setting is required. Strong professional judgment, initiative, and effective time management skills are necessary.
