We are seeking a Social Worker II to join our Central Zone Transition Social Work Team in the Two Hills area. This role provides professional social work services in clients’ homes, continuing care facilities, and hospital settings. The successful incumbent will provide services and work as part of an interdisciplinary team in the following service streams in Two Hills: SAGE, Acute Care, Home Care, and Long-Term Care.

Description:

As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Required Qualifications:

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).

Additional Required Qualifications:

Strong communication and collaboration skills, ability to work independently in diverse settings (homes, congregate living, facilities, hospital) professional judgment, initiative, and time management, proficiency in Microsoft Office Knowledge of seniors’ benefit programs and relevant legislation: Guardianship and Trusteeship Act, Personal Directive Act, Enduring Power of Attorney Act.

Preferred Qualifications:

Experience in Continuing Care settings. Familiarity with Connect Care. Palliative Care experience. Opportunities for professional growth and development.
