Social Worker II
Your Opportunity:
The Glenrose Rehabilitation Hospital Pediatric Social Work Department is seeking a dedicated and motivated Social Worker II to join our team. Reporting to the Pediatric Social Work Manager, this role is an essential member of interdisciplinary teams, supporting children and their families as they navigate the impacts of accident, injury, medical conditions, or developmental diagnoses. The Social Worker II provides patient- and family centered care through consent, assessment, intervention, follow-up, documentation, education, advocacy, resourcing, consultation, crisis management, and discharge or transition planning across assigned programs, units, and clinics. In addition to direct clinical work, the Social Worker II contributes to program planning, resource development, student supervision, evaluation, quality improvement, research, and staff development. This role also participates in initiatives aimed at promoting, maintaining, and improving quality of care and safety for patients, families, and caregivers.
Description:
As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).
Additional Required Qualifications:Additional required qualifications include experience working with pediatric populations and supporting children and families with diverse medical, developmental, and social needs. Candidates must have a strong grounding in ethics and ethical decision making within pediatric and family centered practice, as well as experience working closely with caregivers navigating complex systems. A comprehensive understanding of government systems that support children and families, such as Family Support for Children with Disabilities (FSCD), Child and Family Services (CFS), and Alberta Education. Candidates must also demonstrate strong knowledge of local and provincial programs and services relevant to the children served in our programs, including Program Unit Funding (PUF), the Disability Tax Credit (DTC), and Jordan’s Principle. Familiarity with a wide range of community services, supports, and funding pathways is required to ensure families are connected to appropriate resources.
