Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).
Additional Required Qualifications:Minimum of one (1) year of recent acute care experience (within the past three years) in a hospital setting, preferably on a Medicine or Surgery unit. Strong skills in crisis intervention, comprehensive psychosocial assessment, and complex discharge planning, including experience navigating Continuing Care placement processes. Proven ability to make timely decisions and effectively prioritize in a fast-paced environment. Excellent verbal and written communication skills with strong clinical documentation abilities. Ability to work both independently and collaboratively within an interdisciplinary team. Knowledge of Edmonton-area community resources and referral pathways. Familiarity with harm reduction approaches and experience supporting individuals experiencing houselessness, poverty, substance use, and mental health concerns. Commitment to culturally safe and trauma-informed practice.
Preferred Qualifications:At least two (2) years of experience in an acute care setting, including strong proficiency using hospital electronic medical record systems (e.g., Connect Care).
