Social Worker II

The Social Worker II in the Pediatric Transplant Program is a highly specialized role that supports pediatric patients and families through the transplant process from referral and assessment to transplantation and lifelong post-transplant care. Our multidisciplinary team provides support to transplant recipients ensuring all transplant-related impacts are managed, and overall quality of life is improved. As a member of the Transplant Program at UAH - one of the largest and most established transplant programs in the country - you will contribute to a leading, innovative service that delivers high-quality, patient-centered care. This program is recognized nationally for its clinical expertise, research excellence, and commitment to advancing transplant medicine. In this role you will work collaboratively within a dynamic interdisciplinary environment to provide comprehensive psychosocial assessment, counselling, education, and support to patients/families navigating the complex transplant journey. You will have the opportunity to build meaningful therapeutic relationships over time, engage in complex discharge/ care planning and advocate for equitable access to transplant services. Description:
As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).
Additional Required Qualifications:Minimum of 2 years working as a Registered Social Worker. Strong professional judgement, initiative and effective time management skills are necessary. The candidate will have proven communication skills to work collaboratively and effectively with the health care team members, patients and their families in a fast-paced environment.
Preferred Qualifications:Experience with diverse and medically complex populations; experience working with pediatric patients and families; familiarity with SIPAT. Ability to manage complex situations and navigate community and healthcare systems. The ability to work with various computer programs is an asset.
