Make a meaningful impact on the lives of Albertans within chronic disease management as a team member of the Alberta Healthy Living Program. The community-based program supports adults living with or at risk of chronic health conditions, focusing on improving health and quality of life through patient-centered care. If you are a compassionate and enthusiastic Social Worker who enjoys working with adults, including older adults, this role may be for you. Under general supervision from the Care Manager, you will collaborate with a multidisciplinary health care team, the health system, as well as government and community agencies, to consult, support and advocate for client needs. Your work will help individuals adopt healthy lifestyle habits and improve their health and wellbeing. Key responsibilities include providing generalist social work interventions with clients and their families, responding to and addressing issues that impact the social determinants of health and assessing the instrumental, emotional, and social needs of clients. Psychosocial interventions will include health coaching, supportive and resource counseling, and individual and group psychoeducation. Group education will focus on emotional wellness and self-care. Services are primarily delivered virtually, with some in-person sessions that may require travel during the workday. As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Required Qualifications:

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).

Additional Required Qualifications:

At least three years of experience within the past five years providing patient-centered care, including psychosocial assessments, resource-based interventions, risk screening, supportive and resource counseling, and case management. Recent experience with coordination, consultation and collaboration with interprofessional team members and community agencies. Proficient in delivering both group and one-on-one adult education. Familiar with Alberta legislation related to personal and financial decision-making, advanced care planning, and confidentiality. Strong communication skills—verbal, non-verbal, written, and listening—are essential, along with the demonstrated ability to work collaboratively and independently in fast-paced, evolving environments. Skilled at building respectful relationships with diverse clients, care providers, and team members, and have experience working with culturally diverse and vulnerable populations. Technologically proficient with databases and information systems. Must be available to work occasional evenings and be willing to complete annual training during work hours to attain and maintain certification in Basic Life Support.

Preferred Qualifications:

Completed training and applied experience in Health Change Methodology or other chronic disease management, motivational interviewing, and health coaching strategies. Experience in the development and delivery of virtual and in-person group education. Experience in conducting virtual one-on-one patient appointments. Recent experience with virtual care using Zoom and documentation and workflows in Connect Care. Working knowledge of MS Word and Power Point.
