Your Opportunity:
The Transition Services Social Worker (TS SW) is a part of the multidisciplinary team that provides person centered care for individuals who require facility supported transitional care. The TS SW provides a wide range of services for clients across the health care continuum. The TS SW has experience in the role of advocate and can balance the needs of the individual/family/community with organizational and legislative parameters, including financial accountability for allocation of Alberta Health Services funding to clients. The TS SW also provide clinical psychosocial support for patients and families navigating an often challenging life transition to a continuing care living option, including counselling, crisis intervention, conflict resolution, and mediation. The TS SW conducts ongoing assessment and discharge planning. Clients the TS SW case manages may be assessed for Long Term Care (LTC), Supported Living (SL) or discharged home with home care supports. The TS SW may be required to participate in outreach work with appropriate clients.

As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).
Additional Required Qualifications:This position is a sedentary position and requires long periods of keyboarding and computer use. May be keyboarding up to 6 hour a day. Due to the nature of the work, long periods of time will be spent in a seated position. Two years of recent experience preferably in a Community Health and Acute Care settings. Experience working with geriatric, mental health or complex care adult populations. Previous experience working with an interdisciplinary team.
Preferred Qualifications:5+ years' experience in Mental Health, Home Care, Geriatrics or Transition Services. Community Experience (home care or transition services), Connect Care and Pathways experience.
