Social Worker II
Your Opportunity:
The Social Worker position in this busy rural setting offers a unique and rewarding opportunity to support patients and families across diverse areas of care. This role includes 0.4 FTE at Claresholm General Hospital under AHS, providing services in both acute care and the emergency department (ED), and 0.4 FTE at Willow Creek Continuing Care Centre under ALA. The successful candidate will deliver comprehensive psychosocial support, facilitate discharge planning, and work collaboratively with the interdisciplinary team to ensure seamless transitions and positive patient outcomes. This position is integral to crisis intervention, connecting individuals with community resources, and assisting patients through complex medical, emotional, and social challenges. The ideal candidate demonstrates strong communication skills with a patient- and family-centered approach, along with effective problem-solving, critical thinking, and conflict resolution abilities. A commitment to professional growth, innovation, and continuous learning is essential. The Social Worker actively integrates patient- and family-centered care principles into clinical practice. The position collaborates with other Social Workers in the portfolio for consultations and to provide remote coverage for each other. Claresholm is located approximately 130 km south of Calgary.
Description:
As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).
Additional Required Qualifications:Completion of bachelor’s degree in social work. Registration with the Alberta College of Social Workers (ACSW). Acute care experience within the last year. BLS certification issued within the last 12 months.
