Your Opportunity:

This is an exciting opportunity to join the team at Strathcona Community Hospital (STCH); supporting patients and their families with complex needs. Our site offers a 24/7 Emergency Department, Laboratory & Diagnostic Imaging services, IV Therapy clinic, as well as multiple Ambulatory Care Clinics. Working collaboratively, you will be providing support to patients and their families, across several STCH departments and programs. As a Social Worker II, you will receive internal referrals, and be part of an inter-disciplinary team providing a range of social work services to patients and their families. Our Social Worker may also receive referrals from community partners, and act as a liaison with those partners. Your services may include: psychosocial assessments, resource-based interventions, individual/group/family treatment, risk screening, counselling, case management and coordination, transition/discharge planning, along with prevention/promotion/public awareness activities. This position will focus on patient advocacy, and ensuring patients are linked to appropriate internal and external resources. The successful candidate will be a strong advocate and show professional commitment to learning and growth.

Required Qualifications:

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).

Additional Required Qualifications:

Minimum of 1-3 years’ experience working in a community setting, preference will be given to those with experience working within an Emergency Department. Preference will be given to those who are trained utilizing Connect Care (both ASAP and Ambulatory) for both scheduling and EMR. Excellent interpersonal and communication skills (both verbal and written), and time management skills is required. BCLS Level C – Health Care Provider is required.

Preferred Qualifications:

Strong critical thinking skills, along with demonstrated organizational skills in a fast-paced environment. Demonstrated ability to prioritize work, and effectively work with frequent interruptions, noise, and nearby distractions. Non-Violent Crisis Intervention training an asset.
