Social Worker II
Your Opportunity:
We are looking for a highly skilled and enthusiastic Social Worker to work join our Mental Health and Addiction Intensive Outreach Team in Medicine Hat. Working both independently and as a member of a collaborative multidisciplinary team, you will provide evidence informed and best practices in crisis intervention, assessment, consultation, referral, case management and short-term treatment to clients referred by the courts to the Provincial Mental Health Diversion Program. This position will focus primarily on the mental health diversion programs but may also support other Mental Health and Addiction Outreach programs as required. The Social Worker II works in an integrated and collaborative way to provide treatment for clients with addiction and mental health needs who have been diverted by the courts for low-risk offences. As a member of a multi-disciplinary team, you work confidently, independently, and within collaborative partnerships. You have knowledge of the criminal justice system, mental health and addiction services, resources within AHS and Recovery Alberta and community stakeholders in the South Zone. Your excellent interpersonal skills support your ability to work effectively across a continuum of services. You can balance numerous demands and conflicting priorities, accurately collect, and document information in a timely manner.

As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).
Additional Required Qualifications:Proven organizational and case management skills. Experience working with the criminal justice system. Suicide Intervention training required. Preferred Qualifications:Previous Outreach and/or community experience preferred. Non-Violent Crisis Intervention and BLS preferred.
