We are actively seeking a full time Social Worker II to work on the Active Rehab Unit and within the 12 bed Satellite Program within the Claresholm Mental Health and Addiction Care Centre in the town of Claresholm. Claresholm is a vibrant rural community that is located approximately one and a half hours drive from Calgary and one hour drive from Lethbridge. This position will be responsible for facilitating and coordinating client discharge plans and delivering some group work services in conjunction with the program team. There are two separate Satellite Programs with 6 beds each within the Claresholm Care Facility. This position’s responsibilities are in accordance with the mission, philosophy, objectives and policies to the Claresholm Centre for Mental Health and Addictions (CCMHA). Reporting to the Clinical Operations Manager, this role includes administrative, educational, clinical and liaison activities. The successful incumbent will be expected to be able to demonstrate proficiency in the following areas: Interpersonal skills with the ability to effectively maintain collaborative working relationships with members of a multidisciplinary team. Self-direction and independent decision making and critical thinking skills. Time management and prioritization skills, with a demonstrated ability to function effectively within a dynamic environment. Demonstrated ability to function effectively in a stressful multidisciplinary environment. Effective communication with a patient focused orientation. Creating innovative solutions for complex and diverse issues. Actively integrates patient and family centered care principles.
Description:
As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).
Additional Required Qualifications:Experience in mental health field required. Working knowledge of mental health disorders. Excellent communication, interpersonal and organizational skills. Knowledge and experience providing group therapy. Good computer skills essential including knowledge of the Microsoft Office suite of applications, internet and email applications. Demonstrated commitment to and participation in activities which promote professional and personal growth and development.
