Social Worker II
Your Opportunity:
The Northern Lights Regional Health Centre located in the vibrant community of Fort McMurray is seeking a dedicated Social Worker II to work collaboratively with the Emergency Department team. This role is required to work various shifts, including night and weekends, providing support to complex patients and their families. Your busy shifts will include the facilitation of patients to return to the community by linking them with the appropriate community resource. This will include the early intervention for patients requiring admission and coordinating with various healthcare professionals. Your collaboration and communication skills will be required as work with acute care teams, home care, community partners to ensure comprehensive support for the patients and their families. You will implement and integrated and holistic approach to patient and family centred care, focusing on health promotion and maintenance. As an integral part of the multidisciplinary team, you will be a resource for nursing staff, offering guidance and support based on your expertise and scope of practice. Located within Fort McMurray which resides on Treaty 8 territory, and the beautiful lands of the Metis. In addition to a competitive rate of pay, AHS currently has a Fort McMurray Northern Living Allowance in place to an annual maximum of $12,480. This allowance is non-pensionable and is payable on an hourly basis for all hours paid at the basic rate of pay.

As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).
Additional Required Qualifications:You must be able to work various shifts and have a full license without restrictions, as there is no supervision option in this role.
Preferred Qualifications:Experience with Connect Care will be helpful in this role. Experience within the Emergency Department, acute care, homecare and placement an asset.
