Required Qualifications:

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).

Additional Required Qualifications:

Current BCLS-HCP certification is required. Proficiency in the use of various computer applications including basic word processing, email, internet, electronic charting is required. Must possess strong assessment, communication, organizational, problem solving, counseling and group facilitation skills. Must be able to work independently with a high degree of judgment and must be able to work effectively as part of an interdisciplinary team. Must demonstrate commitment to strengthening team dynamics by actively facilitating interdisciplinary collaboration with internal and external partners to enhance patient care and health outcomes. Strong commitment to patient and family centered care is required.

Preferred Qualifications:

Recent experience working with patients who have complex rehabilitation needs is preferred. Extensive knowledge of community and support services available for patients with complex needs is preferred. Recent experience working in an interdisciplinary team environment is preferred. Experience working with Connect Care scheduling and EMR is considered an asset. Training in the HealthChange Methodology™ is considered an asset.
