Your Opportunity:

This exciting opportunity is support the newly formed corridor based structure in support of the flow of patient repatriations from out of corridor, as well as between facilities within the Central Corridor. The Senior Advisor - Patient Flow position will identify and address processes and/or gaps that may impede patient flow across the corridor.

Description:

Other duties include: These roles will focus on building processes, implementing strategies, and developing best practices to improve patient flow across multiple central corridor sites and other corridors. This role will cultivate relationships with senior leadership and work closely with corridor leaders to resolve barriers that impede patient movement and leveraging their expertise to drive system wide improvements. Support timely repatriation of patients within and from other corridors, complex problem solving, bed capacity management, acute care bed load leveling, engagement of stakeholders and serve as a central point of contact to coordinate planning, exchange of information and consistent messaging across the corridor. Facilitating and enhancing patient flow through utilization of processes to facilitate increased throughput and movement in AHS facilities. Being a single point of contact in the corridor with the knowledge of resources, changes to local conditions and will have the ability to follow up in a timely manner on repatriation or load leveling patients. Demonstrating a strong understanding of the demand and capacity challenges of sites and programs across the corridor focusing on patient satisfaction and leveraging the principle of; patients being in the right place, right time, receiving the right care. Coordinating meetings, managing, evaluating and reporting corridor capacity, patient flow data and information. Collaborating with, but not limited to the ASI Project Lead, site and zone based stakeholders and senior leadership to assist with bed capacity management. Advocating for patient centered care in alignment with AHS Foundational Strategies.

Required Qualifications:

Bachelor's Degree in a health profession. A minimum of 5 years of recent diverse (acute / community) clinical and/or managerial healthcare experience. 5 years registration with a health profession body. Reside within the Central Corridor.


Additional Required Qualifications:

Proficient in using Microsoft Office Suite including Teams. Proficient in the use of Connect Care RAAPID tracking boards. Strong communication skills – verbal, written and virtual in a variety of settings. Experience in implementing and sustaining new programs. Demonstrates ability to develop effective relationships with a variety of stakeholders. Exhibits proficiency in time management and prioritization skills. Strong critical thinking, problem solving and communication skills. Strong clinical skills and experience with bed management and patient flow processes.
