Your Opportunity:

Reporting to the Unit Manager, the Central Access and Triage (CAT) RN is responsible for coordinating the overall functioning of the central referrals office and triage process. The RN reviews incoming patient referrals, assess urgency and appropriateness of referrals, triages patients to specific physicians to ensure timely appointments are created based on patient needs (under direction of orthopedic surgeon). The RN acts as a liaison between the central access office and the department of Orthopedic Surgery and liaises with Primary Care Physicians and referral agents to facilitate the referral process. The triage Nurse will collaborate, manage conflict and provide leadership & oversight to the referrals office that triages referrals for 16 Surgeons in 4 clinics (Hand, Cast, Hip & Knee, General Orthopedic). The RN assists in triaging and processing incoming /outgoing referrals, monitors appointment work ques, as well as monitoring surgeons wait-times to ensure wait times to referrals are assigned to appropriate clinic and surgeon. The RN will collaborate with and provide monthly waitlist statistics to both the Physician leads of Hand and Orthopedic Surgery as well as the unit manager to decide on next available physician referral acceptance. The RN collaborates with the Surgeon, arthroplasty RN case managers and medical office assistants (MOA’s) in delivery of care. The RN will also monitor clinic in basket pool for messages, online RightFax for incoming documents/referral not received by the clinic to ensure items are actioned in the fax folder. The triage RN will assist in revieing clinics policies and procedures, updating and archiving as appropriate. The Triage RN is required to be onsite fulltime, Monday - Friday, from 8:00 am - 4:00 pm and fulfill the Nurse Clinician role whenever the clinician is absent.

You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values of AHS. PLEASE NOTE: Provisional Permit Holders shall be paid $39.58 /hour - $54.16/hour. Upon being entered into the register of Registered Nurses, the rate of pay shall be adjusted to the rate of pay applicable to a Registered Nurse in accordance with AHS/UNA Article 25.02(b)(i) or (ii) as applicable.

Required Qualifications:

Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP). If you have not practiced as a Registered Nurse within the last five (5) years, completion of the following free courses is required: CRNA Medication Management - Chapters 1 - 4 (https://connect.nurses.ab.ca/home/%2Flearning-and-development%2Flearning-modules%2F) and the NextGenU Health Assessment Resources Certification (https://courses.nextgenu.org/course/view.php?id=271#section-0)

Additional Required Qualifications:

Minimum 5 years of recent acute care experience. Minimum 3 years of recent acute care orthopedic nursing experience. Minimum 1 year experience using EPIC in Ambulatory clinical setting Minimum 1 year experience Ambulatory Referrals Management & Waitlists. Strong understanding of referral entry and waitlists. Experience with managing referral Workques 1 to 5 in Connect care (1- Action Needed, 2-Awaiting Triage, 3-Ready for Scheduling, 4-Pending notification, 5 -Outgoing Referrals). Demonstrated strong interpersonal and decision making skills. Demonstrated critical thinking & assessment skills and ability to problem solve. Proficiency using Connect Care & Microsoft office (excel, word, outlook, sharepoint, PowerPoint). Excellent time management, organizational skills and manage multiple priorities.

Preferred Qualifications:

Bachelor's of Nursing Degree. Knowledge of Orthopedic diseases, disorders, procedures and treatment protocols. Previous case manager / nurse clinician experience. Telephone assessment and triage experience. Experience with generating and presenting data. Current experience with hip and knee care path preferred. Understanding of the Alberta Referral Directory.
